Can I get relief for severe ear and gum pain?

I'm 66 yrs old I am having a ear ache and I have a knot under my gum the both of them is giving me a headache and hurting till I can't sleep I've taken over the counter medicine applied heat to my face and nothing is helping my whole face and head hurts need something for this can't take this pain anymore
